Difficult Hyperpigmentary Disorders: An Asian Perspective

Hyperpigmentation, a common presenting complaint among Asians, can due to increased melanogenesis, melanocytosis, as well as delayed breakdown and removal of melanin. Although hyperpigmentary disorders can be broadly categorised into congenital/genetic or acquired causes and present as diffuse or localised forms, certain hyperpigmentary dermatoses peculiar to Asians are difficult to classify. This can arise because of paucity of description in the literature, inadequate elucidation of the clinical spectrum of the disorder, as well as overlapping or non-specific pathological findings - making accurate diagnosis difficult. In this presentation, hyperpigmentary disorders such as lichen planus pigmentosus, erythema dyschromicum perstans, idiopathic eruptive macular pigmentation, amyloidosis cutis dyschromica and “mixed” melasma will be highlighted and the controversies surrounding their diagnosis and classification discussed.
